Abstract

L'érysipèle est une infection dermo-hypodermique bactérienne aiguë, de siège relativement superficiel. Sa tendance à la récidive dans la même région est fréquemment favorisée par le lymphoedème chronique. Nous présentons une observation assez caractéristique d'érysipèle, mais particulière par ses aspects cliniques et histologiques initiaux trompeurs, sa localisation inhabituelle en regard de la hanche gauche avec extension loco-régionale et son caractère récidivant. En effet, cinq épisodes similaires sont survenus en 5 mois en regard de la prothèse totale de hanche, mise en place 9 ans et demi auparavant. Le taux des antistreptokinases est augmenté à 320 U.ASK/ml. L'évolution a été favorable sous amoxicilline et ultérieurement antibio-prophylaxie par Extencilline Il s'agit donc d'un érysipèle récidivant, secondaire aux remaniements lymphatiques liés à la pose de la prothèse totale de hanche. La revue de la littérature incite, devant ce type de tableau clinique, à une surveillance prolongée afin de s'assurer de l'absence ultérieure soit d'infection prothétique, soit de dermite d'intolérance au matériel prothétique.
